The story starts exactly on November 9, 1988. Charles Lee Ray, the notorious "Lakeshore Strangler", is shot and mortally wounded by Federal Detective Mike Norris while fleeing the police. Before dying, Charles takes cover inside a toy store with a liquidation of the Play Pals "Good Guy" dolls and uses a voodoo ritual in order to transfer his soul into one of the dolls. The store is then struck by lightning and it burns to the ground.
Later that morning, Andy Barclay and his mother, Karen, are introduced. It is Andy's 6th birthday and all he wants is the Good Guy doll he has been seeing in television commercials. However, his mother couldn't afford the doll and got him clothes instead. Realizing her son's disappointment, she later buys a Good Guy, the same hellish doll containing the soul of Charles Lee Ray from a homeless peddler.
Charles Lee Ray (or "Chucky", as he identifies himself to Andy) soon reveals to Andy his identity and his true nature, and a series of deaths begins. Chucky kills Karen's friend, Maggie. The next day, Andy and Chucky visit an accomplice of Charles Lee Ray and Chucky kills him as well. Andy is placed in a psychiatric hospital after he is found near the scene. Karen suspects the doll after she finds that his battery compartment is empty and she contacts Mike Norris, the same detective who killed Charles Lee Ray and then was assigned to investigate Maggie's death. Although he initially doubts her story, the homeless man who sold Karen the doll confirms that he retrieved it from the burnt remains of the toy store where Charles Lee Ray was shot and killed. Norris becomes a believer after he is attacked by Chucky in his car and barely survives.
Meanwhile, Chucky has met with John, his voodoo instructor, and discovered that, in order to escape his doll body, he has to transfer his soul into the first person that he told his true identity, Andy. Chucky injures John and leaves him to die. Karen and Norris, following leads from Charles Lee Ray's case file, discover John prior to his death and receive instructions on how to kill Chucky. Although he is in a doll body, his heart is a human heart and he can be injured there.
Chucky goes to the hospital and frees Andy, killing a doctor in the process. Andy escapes with Chucky and goes back to his apartment. Karen and Norris, hearing that Andy has gone missing, hurry back to the apartment in the hopes that Andy will go there.
Norris and Karen confront Chucky in the apartment just as he is preparing to transfer his soul into Andy's body. After a prolonged fight, Chucky is thrown into the fireplace and Andy sets him on fire. Andy and Karen, thinking the danger is over, go to help the injured Norris. Chucky, however, crawls out of the fireplace. Karen shoots him several times, missing his heart, but scattering his head and limbs around the hallway. After another police officer arrives, the scattered doll parts attempt to attack at the instructions of Chucky's shot off head, but Norris finally shoots Chucky in the heart, destroying Charles Lee Ray for now
